This means there is a gap between the lid and the section dividers in the case. If a pill is small enough, it can fit between the lid and the top of the section divider and get into another section. Plus, the whole point of an organizer is to organize. When they're all together in one section, they are not organized. I loved the case and wanted to keep it so I problem solved a solution that works fine. I cut out a circle of think cardboard (i.e. the thickness of a box of tea, which is what I used, actually) and placed it on top of the divider. It's smaller in diameter than the outer metal case and doesn't stick out. It must be thick enough to do the job because I don't have any other spills. My tea cardboard had soft pink roses on it, so I'm actually kind of happy because it looks nice with the brass. The fact that you might have to rig up a system like mine is not a problem because the pill case is so nice. You don't see the cardboard on the outside, so the cardboard doesn't affect the look of the case. If you have large pills, you won't even need to cardboard. Test it and see before you decide. I've tried a handful of different pill cases in the past that all had durability issues, or would simply open by themselves too easily. This one has been in my pocket for probably six months now. I was first drawn to how simple and rustic this one is. It's a little thinner than some of the other ones I've had, which I like because it fits more easily into the pocket watch pocket in my jeans. If it ever does break, I will definitely buy it again. My only caution would be, if you have medicine that could be harmful to kids, be sure to keep this out of their reach because it is not hard to open. (Monkey see monkey do).
