You put your mouth over the mouthpiece and breathe in and out of your mouth quite slowly, but quite forcibly, with each in/out representing 1 breath, so aim for 30 of these per day. Start with a light tension, increase the tension as you start to find each 30-breath cycle easier. Tension is increased by turning the arrow which is on the handle, to tighten the coiled spring inside it - so that it is aligned with a higher number on the handle. I started at 2 and ended up as far as 6. I cleaned the whole device in sterilised water once a week and the clear plastic mouthpiece every alternate day.
