First off, no pack is going to feel good on a sore jaw or face, but that is NOT the point. The point is to be able to hold ice to your face for 20 minutes at a time and it works great for that. If it is too tight, just loosen the band.I have bad TMJ and I am using this ice pack and botox injections in the jaw muscles to relieve most of it. The ice should be as cold as you can stand it for 20 minutes at a time. No more than that. Do it for 1-3 times a day and let your jaw warm to room temperature after using before talking a lot or eating food. Some people were mentioning that its too cold to put on your skin but I just put a thin tea towel between the pack and my skin and it was perfect and then when it started warming up I just removed the towel to get another burst of cold before I had to refreeze it. You can even put other gel ice packs you already own in the holder and use it while the one it came with is re-freezing. They don't fit quite as nice but they work just as well. This was ok, but really it barely fit my 15 yr olds head. In fact I had to use a tissue to protect her hair from getting caught from the velcro. She has a normal sized head...so they need to make the strap longer. Also, a much better design would be for their to be separate ice packs to slide in that way you could keep a set on hand to swap out.
