Is there a good way to extend the output leads? I need them to be longer than they currently are.

Strip the outer end of the wire about 5mm and solder on longer wires. Then heatshrink the joined part for insulation.

i beleive it is 3 volts dc to an inverter with high volts AC out not DC out

AC. Probably around 40 000V peak to peak. Low amps.

How can it be 400kv if the output leads are only 1 inch apart and use ordinary 250v insulation? if it was more than 25kv it would just arc the leads

Well, first of all, that's not ordinary insulation. I didn't test it, but it looks like kynar. The maker used good output wiring. I'm not going to back his claim of 400KV, because I personally have not taken one of the units into my lab and measured it, but I know a good bit about HV. Don't assume that the breakdown characteristics of air are linear. They're not. Under normal kitchen table conditions, you can draw 4 inch sparks from a 20kv source. Once a plasma conduction path is established, it is self-sustaining as long as the current keeps it hot.
